For devotion to duty and outstanding leadership. His ship's company were outstanding in their behaviour under air attack and during long periods of successful minesweeping at Tobruk. He also carried out very good work during the evacuation of Solum (Egypt) 11th and 12th April 1941.

Lieutenant Thomas Michael Godfrey Codrington was born on 7th December 1917 at Grimsby. He was the son of Commander G.G.Codrington, Royal Navy and related to Sir Admiral Sir Edward Codrington of Navarino fame after whom a destroyer was named in 1929. He was educated at Pangbourne Nautical College and entered the Merchant Navy as 4th Mate on 30.6.1939 and served aboard SS Remucra owned by the New Zealand shipping company. He joined the RNR as a probationary Midshipman 1.2.1935 with Seniority 1.1.1935, Midshipman 14.3.1938 with same seniority. Commissioned acting Sub Lieutenant 3.2.1939 and promoted Lieutenant 16.7.1941. He joined HMS Drake on 13.8.1939 for Gunnery course, HMS Victory for minesweeper training and the Laguna Belle 10 October 1939 becoming its First Lieutenant 15.5.1940. He joined Vernon 26.8.1940 for further minesweeping training and then to the Mediterranean where he was awarded the DSC for services aboard HMS Soika.

He joined Medway for Submarine course 4th September 1941 and HM Submarine Perseus 11.9.1941. The Perseus sank the German Castellon in the Gulf of Sirte in October 1941 and on 26th November 1941 left Malta to patrol the Ionian Sea.

H.M.S. Perseus struck a mine in the Ionian Sea on the night of 6/7 December 1941, laid by the Italian submarine Enrico Toti. Lieutenant Codrington was killed with this and is commemorated on the Porstmouth Memorial.

The submarine had one survivor a Leading Stoker J.H.Capes who swam 6 miles to Cephalonia and eventually reached allied lines via Turkey, he was awarded a BEM for his escape.
