Folio (271 x 185 mm). A-C6, D6[-1]. Woodcut border to title, woodcut initals. Modern calf over marbled boards. Lacks colophon leaf, dampstained, a few tiny wormholes, tears mainly to margin of D5.
This edition with D5v, line 3 begining with "ful," and line 4 has "hym." STC 9478.3.
